Your First Visit

Please arrive to your appointment 5-10 minutes early.

When you come for your first visit please remember to obtain and bring the following

• Referral letter from GP, family physician or another doctor
• Medicare card, DVA card, pension card
• Have your Private Hospital Insurance information with you
• Images and reports for X-rays, MRI's, CT scans etc. and any other investigation that your doctors have ordered for you
